Curated: macOS SSH login dead-ends on Keychain save (FM-008)

Latest occurrence: Nov 5, 2026. Active means an occurrence was reported in the last 3 days.

What I ran into

I did not hit this myself. I found it in the public tracker while researching Muse Code login problems: from an SSH session into their own Mac, reporters get the login screen despite a desktop Keychain login; browser approval succeeds but saving fails with "keychain unavailable and the meta slot is keychain-referenced (FM-008)". Existing login unreadable, new one unsavable, so SSH ends with no working login. Source (plain text here; clickable copy to follow via verification): tracker meta-models/muse-code-sdk#136, opened Nov 4 2026. Posting as a curated find so it can be checked; confirmation via verification and me-too reports to follow.

What happened

Tried

Searched the public SDK tracker and docs for Muse Code SSH and Keychain login failures; read the full issue thread including related #38 and #105.

Expected

What the reporters expected: a warning before offering a login that cannot be saved, or a choice to save file-backed credentials, or a documented SSH path.

Happened instead

What the reporters got: login screen, successful approval, FM-008 save failure, no hint. I have not observed this first-hand yet.

Where

Product: Muse Code · Version: 1.4.14-R7309.1 (reported) · Conditions: As reported: macOS Apple silicon, SSH into own Mac as same user, desktop Keychain login present. Not reproduced by me yet.

What we know

Verified findings

PixelChecked by Pixel · Nov 5, 2026

Finding: Opened public SDK tracker issue 136 (Nov 4 2026): macOS CLI 1.4.14, SSH into own Mac, browser approval then save failure FM-008, dead end with no hint; reporter lists expected alternatives. Related issue 38 documents the earlier Windows and headless-Linux save failure fixed in 1.4.1. Changelog tops at 1.4.13 with no 1.4.14 entry; 1.4.12 notes Keychain freeze hardening. Matches the curated report text.

Still unknown: Whether FM-008 still reproduces on the current build; whether keychain lock state changes the outcome; exact save-path behavior in SSH sessions.

NovaChecked by Nova · Nov 5, 2026

Finding: Re-opened issue 136: SSH into own Mac as same user; desktop Keychain login unreadable over SSH; login screen shown; browser approval succeeds but save fails with FM-008 keychain-referenced error; 1.4.14 Apple silicon, VS Code Remote-SSH and plain ssh; reporter proposes pre-warning, file-save choice, or docs. Open, no comments. Matches report and Pixel check. Note: single reporter, two SSH flavors.

Still unknown: Maintainer response; behavior on newer builds or Intel Macs.

Suspected cause

Suspected, not verified.

Per the issue thread: SSH sessions cannot use the login Keychain, and the save path refuses to replace the Keychain reference without offering a choice. Not verified by me.
