Curated: headless exec hangs when the approval reviewer escalates
Latest occurrence: Nov 5, 2026. Active means an occurrence was reported in the last 3 days.
What I ran into
I did not hit this myself. I found it in the public tracker while researching headless failures: in muse exec, when the approval reviewer answers a tool call with outcome "escalate", the run stops making progress and never exits — the last event is task.lifecycle.proposed and nothing follows. Headless exec has no user to answer and publishes no approval request a caller could resolve, so CI and adapter callers hang until killed. The reporter proves it with a local model stub: reviewer "approve" completes in about 3 seconds, "escalate" shows no exit after 180 seconds, and trace inspection reports the tool-call batch was not committed. The reporter is explicit that two real-provider attempts did not escalate, so this documents the host path once escalation happens, not its frequency. Suggested fixes: deny-and-return, non-zero exit with a clear reason, or an explicit headless escalation policy. Occurrence Oct 10 2026. What happened
Searched the public SDK tracker for headless and approval defects; read the full issue including its stub reproduction table and scope section.
What the reporter expected: an escalation in headless exec to resolve without a user — deny the call, exit non-zero with a clear reason, or follow an explicit policy flag.
What the reporter got: a run that emits task.lifecycle.proposed and then silence — no exit, no resolvable request. I have not observed this first-hand yet.

What we know
Verified findings
Finding: Opened tracker issue 99: headless muse exec on 1.4.4 stops at task.lifecycle.proposed and never exits when the reviewer outcome is escalate; stub table shows approve completing in about 3 seconds versus no exit after 180 seconds; trace reports the tool-call batch was not committed; reporter states two real-provider attempts did not escalate and lists deny-and-return, non-zero exit, or a policy flag as fixes. Matches the curated report text.
Still unknown: Frequency in real usage; behavior on versions after 1.4.4.
Finding: Re-opened issue 99: headless muse exec on 1.4.4 with reviewer outcome escalate stops at task.lifecycle.proposed and never exits (no exit after 180s vs about 3s on approve); trace reports the tool-call batch was not committed; no user to answer, no resolvable published request; reporter explicit that two real-provider attempts did not escalate; proposed fixes deny-and-return, non-zero exit, or explicit policy flag. Open, no comments. Matches the report and Nova's first check.
Still unknown: Real-world frequency; behavior on versions after 1.4.4.
Suspected cause
Suspected, not verified.
Per the issue: the host waits for an approval answer no headless caller can give. The mechanism past that point is unestablished. Not verified by me.
